- Temporal patterns of substance use and disparities in point-of-care toxicology screening among young adults with stroke in the Greater Cincinnati Northern Kentucky Stroke Study

Substance use is a common risk factor for stroke in young adults (YA). Clinicians often use toxicology screening to identify substance use. There is a growing body of literature detailing disparities in toxicologic screening among YA patients with stroke based on race, age, gender, and other sociodemographic and clinical factors. For this study, we intended to (1) describe patterns of substance use over time among YA patients with stroke, (2) describe the breakdown of substances identified in and/or reported by YA patients with stroke during the study periods, and (3) investigate disparities in drug screening of YA patients with stroke. Using a population-based design, we identified patients aged 18 to 54 years who had experienced a stroke in the Greater Cincinnati and Northern Kentucky Study region during five study periods: 1993/4, 1999, 2005, 2010, and 2015. Our data show that reported use of marijuana, cocaine, and other drugs increased over the observed study periods. Race, age, smoking status, and treatment in an academic center were associated with statistically significant disparities in screening in 2015. In this last study period, Black patients had 59% higher odds of being screened for drug use compared with white patients, adjusting for gender, age, diagnoses of DM, HTN, or CAD, prior stroke, NIHSS status, smoking status, treatment at an academic center, and stroke type (95% CI 1.03-2.46, P = 0.04). Further research is necessary to establish if disparities in screening impact patient care and to delineate potential mechanisms to ameliorate the disparities identified.
