     <mods:abstract>Traditional explanations of health system change have tended to focus on such factors as institutional configurations and provider power. I posit the potency of patients as
      citizens in the shaping of health care reform. Not only does the absolute strength of patients within countries matter, but the perception of patient desires and needs on the part of the State
      is equally important in determining the course of change. This narrative does not supplant traditional explanations, but instead seeks to add to them, thus enhancing our understanding of the
      conditions that tend to systemic alterations in health care. I employ the United States and United Kingdoms as the key cases under consideration, and compare (and contrast) three periods of
      reform--or near-reform--in the health care systems of both countries.</mods:abstract>
