A Quantitative Analysis of the Charlson and Hopkins Risk Score to Better Predict High-Risk Patients in a Behavioral Health Population
Abstract
Hospital readmissions is a growing crisis within the US that presents with increasing hospitalization rates and per capita costs. For adults with mental illnesses, approximately half of all patients discharged are readmitted within a year. Mental health disorders and substance use disorders are amongst the top five-diagnoses associated with a 30-day readmission. One possible solution is transition-of-care programs that provide care to recently discharged patients. The Charlson Index and Hopkins ACG are risk scores that predict mortality and hospital readmissions, indicating which patients are “high-risk”. This study aimed to determine which risk score better predicted “high-risk” patients to help guide transition-of-care programs in reducing readmission rates.
