   <mods:abstract>The US Centers for Medicare and Medicaid publishes Five Star Quality Ratings on the Nursing Home Compare (NHC) website to serve as a quality assessment tool for Skilled Nursing Facility (SNF) residents and families. The reliability and efficacy of these ratings is publicly debated, and the association between five star ratings and clinical outcomes is unknown. Past studies report conflicting findings and inconsistent data regarding the association between these ratings and clinical outcomes. Additional research on the NHC Five Star Rating System is necessary to inform patients, families, and physicians in their decision-making process when selecting a SNF. We conducted a retrospective chart review of patients 65 years or older treated at RIH for primary diagnosis of hip fracture who were discharged to SNF. We compared NHC Five Star Ratings to one year morality, hospital readmission, and ED visits. Of the 245 patients discharged to SNF, 122 were discharged to SNF with 1-3 Overall star rating, and 123 were discharged to SNF with 4-5 Overall star rating. CMS Staffing rating was associated with an increase in ED visits. Patients discharged to SNF with 4-5 star Staffing Rating were more likely to have at least one ED Visit than 1-3 star SNF. This study observed variable trends among domain specific ratings, and there was no significant association between CMS overall Five Star Ratings and one year outcomes</mods:abstract>
