Printed in two columns divided by thick black line; within mourning border with memento mori (Reilly 31) centered on top; title and colophon boxed.
Two angel type-ornaments (Reilly 726) at end of text in second column.
At bottom, within two thick lines: Boston: Printed and sold at the Printing-Office in Milk-Street. MDCCLXX.
Internal evidence of woodcuts and address may indicate the firm of John Kneeland and Seth Adams as printers.
