Addressing the Social Determinants of Health Affecting Formerly Incarcerated Patients and Reducing Risk for Opioid Overdose at Thundermist Health Center

Abstract:
Former inmates have a risk of death two weeks following release at a rate 12.7 times higher than the general population, with the leading cause of death being overdose, making this a crucial time period for intervention. This quality improvement project brings attention to the particular SDH affecting the formerly incarcerated patient population, as well as the heightened need to distribute life-saving naloxone.
