Quality improvement project to identify patients at high risk for trauma using a universal social determinants of health screen in a community health center setting

Abstract:
All people experience and/or witness at least one traumatic event in their lifetime which can impair their ability to function productively and socially. Because federally qualified health centers (FQHCs) serve vulnerable populations, identifying those who are experiencing any form of trauma is essential. Our objectives were to identify patients experiencing trauma with a social determinants of health (SDH) screen “hurt question” and propose a trauma screening tool to identify, characterize, and monitor patient trauma.
