- Vasomodulatory Dysregulation after Cardiopulmonary Bypass with Uncontrolled Hypertension

- Abstract
- Vasogenic dysregulation after cardiopulmonary bypass is a well established phenomenon. Furthermore, this dysfunction is upregulated in patients with uncontrolled hypertension compared to normotensive patients. The mechanism behind this requires further elucidation, however there remains a significant gap in understanding as to which genetic changes that occur after cardiopulmonary bypass my contribute to this change, and how differences in hypertension status may regulate these changes. In this study, we compared RNA sequencing data from patients pre and post cardipulmonary bypass to determine which genes were more or less regulated by patient population (uncontrolled hypertension N = 5, controlled hypertension N = 5, normotensive N = 2). In total 109 genes were upregulated in controlled hypertension compared to baseline, 46 genes in uncontrolled hypertension compared to baseline, and 31 genes in controlled hypertension compared to uncontrolled hypertension. The majority of upregulated genes in hypertensive patients, regardless of control, were proinflammatory or assisting in angiogenesis or other forms of vascular remodeling, possibly indicating changes to the endothelial structure that may change responsiveness to vasomotor stimuli. Interestingly, several proinflammatory markers were upregulated in controlled hypertension compared to baseline. Further research with greater sample size (particularly for normotensive patients) is necessary to confirm the target genes and investigate the contribution of specifically identified candidates to vasogenic dysregulation.
