<mods:mods xmlns:mods="http://www.loc.gov/mods/v3" xmlns:xlink="http://www.w3.org/1999/xlink" xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" xsi:schemaLocation="http://www.loc.gov/mods/v3 http://www.loc.gov/standards/mods/v3/mods-3-7.xsd"><mods:titleInfo><mods:title>The Impact of Alcohol Use Disorder on Blood Transfusion Outcomes in Trauma Patients</mods:title></mods:titleInfo><mods:typeOfResource authority="primo">text_resources</mods:typeOfResource><mods:abstract>This study aims to investigate the influence of alcohol use disorder (AUD) on clinical outcomes among trauma patients receiving blood transfusions in the emergency department at the University Teaching Hospital of Kigali (CHUK), Rwanda. Specifically, it will examine how AUD affects mortality rates, ICU admissions, length of hospital stays, and the volume of blood transfusions. Additionally, the study will analyze demographic data (age, sex, geographic residence) to identify vulnerable populations and explore correlations between physiological markers such as prothrombin time, hemoglobin levels, and lactate levels with clinical outcomes.
This single-center prospective observational study focuses on trauma patients aged 16 and older who are administered blood transfusions at CHUK ED. Research assistants approach eligible patients shortly after admission for enrollment. Participants provide informed consent and complete the AUDIT-C screening survey, translated into Kinyarwanda. Data collected from health records includes demographics, cause of trauma, injured body parts, time of arrival, Glasgow Coma Scale (GCS) scores, intubation status, and details of blood transfusions received. Key outcomes of interest are mortality, ICU admissions, and length of stay in the emergency department.
The hypothesis posits that trauma patients who screen positive for AUD will have higher mortality rates, increased ICU admissions, prolonged hospital stays, and greater transfusion requirements. This assumption is based on the premise that individuals with AUD engage in higher-risk behaviors, leading to poorer clinical outcomes.
Existing literature on trauma patients receiving blood transfusions, both with and without AUD, is limited. Research indicates that ethanol consumption may elevate plasma lactate levels and presents conflicting evidence regarding prothrombin time in AUD patients. While some studies suggest that alcohol consumption prolongs prothrombin time and hampers clot formation, others find no significant effect. Additionally, lower hemoglobin levels have been observed in patients with moderate to severe alcohol use.
The study received IRB approval on August 25, 2024. As of now, three participants have been enrolled. Notably, the recent epidemic of Marburg virus disease in Rwanda, primarily affecting healthcare workers, poses additional challenges to data collection.
