Collagen-Embedded Cesium-131 After Metastasis Resection: Local Control in Newly Diagnosed Brain Lesions

Abstract
Objective: While cesium-131 collagen tile has gained traction as a platform for radiation delivery to recurrent intracranial tumors treated with surgical resection, outcome data in the setting of newly diagnosed brain metastases (nBM) remain sparse, with fewer than ten patients reported across published studies. This study evaluates the safety and efficacy of cesium-131 collagen tiles in patients with nBM. Methods: Clinical information was collected for consecutive nBM patients treated at three neurosurgical centers. Actuarial local control and overall survival (OS) were calculated from the time of cesium-131 collagen tile implant. Results: The cohort comprised 14 men and 22 women (mean age: 62.1+ 8.9 years) and 39 nBM. The histology included: 19 lung, 8 melanoma, 4 breast, 3 gastrointestinal, and 5 genitourinary metastases. The mean maximum diameter of the pre-operative BM was 3.29 cm (+1.35 cm). Gross total resection was achieved in all but one subject. The mean Karnofsky Performance Score (KPS) before resection/tile implant was 81 + 11.1. There were no perioperative complications, with the average hospital stay of 2.2 + 1.6 days. There were nine 30-day readmissions (25.0%) and seven 90-day readmissions (19.4%). With a median follow-up of 328 days, the actuarial local control rate at one year was 100%, with one local failure two years post-implant. The median OS of the cohort was 182 days. There was one case of radiation necrosis (2.6%). In terms of functional status, KPS improved or remained stable for 28 patients and worsened in 8 patients (22.2%). At the one-month follow-up, 8/36 (22.2%) patients showed persistent KPS decline relative to the pre-operative status, which was associated with shorter OS (65 vs. 211 days, p=0.02). Conclusion: This study represents the first dedicated evaluation of cesium-131 collagen tiles in the setting of nBM, demonstrating a favorable safety and efficacy profile. The series provides a foundation for future validation and clinical applications.
