Automating Capsulotomy Size Measurement in Femto Laser-Assisted Cataract Surgery with Computer Vision

Abstract
Femtosecond Laser-Assisted Cataract Surgery (FLACS) relies on the creation of a precise and well-centered capsulotomy to ensure optimal intraocular lens (IOL) placement and postoperative outcomes. Inaccurate capsulotomy sizing may compromise IOL centration, increase the risk of posterior capsular opacification, or complicate advanced techniques such as reverse optic capture. Traditional measurements of the capsulotomy diameter often assume a flat plane, disregarding the natural curvature of the anterior lens capsule. In patients with thicker cataracts, increased lens curvature can further amplify discrepancies, resulting in larger capsulotomy sizes. This study demonstrates that a computer vision model can automatically precisely measure arc length, enabling more personalized and accurate capsulotomy measurements. Physicians should also be mindful of potential errors in FLACS capsulotomy diameters, particularly in patients with greater lens thickness.
