At PACE Rhode Island, a program supporting elders in living safely at home, fracture prevention is essential. In Rhode Island, the average post-fracture cost for Medicare beneficiaries exceeds $44,800 per person, more than double the national average. Fractures also severely impact quality of life through pain, complications, and loss of independence. Certain medications can increase fracture risk by reducing bone mineral density (BMD) or contributing to falls, but prescribers may be unaware of these effects. This project aimed to create an accessible tool to help PACE-RI clinicians assess medication-related fracture risks and apply mitigation strategies.&#13;

Methods&#13;
A literature review evaluated the 50 most prescribed medications at PACE-RI. After removing duplicates with variable dosing, drugs were categorized as: Increased Risk (evidence of BMD reduction), Potential Risk (inconclusive/conflicting evidence), Beneficial/Protective (evidence of BMD preservation), or No Known Impact. Medications were also flagged if they increased fracture risk through secondary mechanisms, such as dizziness or syncope. The findings were compiled into a Bone Health: Stoplight Guide to Medications at PACE, providing visual categorization and risk-mitigation suggestions (e.g., calcium supplementation).&#13;

Results&#13;
Among the 30 medications analyzed, 27% were classified as Increased Risk, 17% as Potential Risk, 13% as Beneficial/Protective, and 43% as No Known Impact. Half the medications were linked to increased fracture risk due to secondary effects. The Stoplight Guide was developed as an educational tool to improve prescribing practices and fracture prevention.&#13;

Conclusion&#13;
This study highlights the need for greater awareness of medication-related fracture risk in older adults.
