Characterizing Trends in Disenrollment Among Medicare Advantage Enrollees with Functional Impairment

Abstract:
Background: Previous research suggests that Medicare Advantage (MA) may be successful in improving patient outcomes3 but patients with greater healthcare needs tend to leave plans at higher rates, indicating that plans may not be meeting the needs of these groups.4-6 Objective: The objective of this study is to develop a better understanding of disenrollment and plan switching in MA among those with varying levels of functional impairments. Design and Participants: This cross-sectional study of MA enrollees from 2015 to 2017 used multivariate linear regressions stratified by Medicare-Medicaid eligibility status. The total sample consisted of 630,117 enrollees who completed the Health Outcomes Survey (HOS). Main Measures: The proportion of enrollees who disenrolled into TM, or who switched plans within the MA program. Key Results: Beneficiaries with severe impairment had a 0.58 percentage points (pp) higher probability of disenrolling to TM (95% CI: 0.41, 0.75), while those with moderate impairment had a 0.16 pp higher probability of disenrolling (95% CI: 04, 0.28) compared to beneficiaries without functional impairment. Beneficiaries with severe impairment had a 1.20 pp higher probability of switching plans within MA (95% CI: 0.81, 1.59), while beneficiaries with moderate impairment had a 0.98 pp higher probability of switching (95% CI: 0.71, 1.25). Duals with moderate impairment had a higher probability of switching within MA, and duals with severe impairment had higher probability of disenrolling. Non-dual beneficiaries with moderate and severe impairment had a higher probability of switching within MA or disenrolling to TM. Conclusions: Community dwelling individuals with higher degrees of functional impairment were more likely to disenroll from or switch their MA plan. This result suggests that functional status may be important to consider regarding MA plan selection and delivery.
