To evaluate the association between dietary quality, as measured by the 2015 Healthy Eating Index (HEI-2015), and risk of incident rheumatoid arthritis in postmenopausal women.&#13;

Methods:&#13;
109,591 postmenopausal women without baseline RA were prospectively followed as part of the Women’s Health Initiative for an average of 8.1 years. Baseline diet was measured using an FFQ and diet quality was evaluated using the HEI-2015. Multivariable Cox proportional regression analysis was performed evaluating the association of diet quality with self-reported physician diagnosed RA after adjusting for age, race, education status, income, and BMI. An additional model adjusting for all potential confounders was evaluated.&#13;

Results:&#13;
During 1,241,532 person-years of follow-up, 5,823 incident RA cases were identified. Compared to quartile 1,  quartiles 2, 3, and 4 of the HEI-2015 scores were associated with hazard ratios of 0.97 (0.90, 1.04), 0.87 (0.81, 0.94), and 0.82 (0.76, 0.88), respectively (p-trend &lt;0.001) (Table 3). A higher score for total fruits (HRQ4 = 0.90, 95% CI: 0.84-0.98, p-trend=.006), whole fruits (HRQ4 = 0.86, 95% CI: 0.80-0.92, p-trend&lt;.001), total vegetables (HRQ4 = 0.89, 95% CI: 0.82-0.96, p-trend=.004), dark green vegetables (HRQ4 = 0.86, 95% CI: 0.80-0.93, p-trend&lt;.001), whole grain (HRQ4 = 0.89, 95% CI: 0.83-0.97, p-trend=.004), total dairy (HRQ4 = 0.89, 95% CI: 0.81-0.97, p-trend=.004), and seafood/plant protein (HRQ4 = 0.90, 95% CI: 0.83-0.98, p-trend=.015) consumption was significantly associated with lower rates of incident RA independent of age and confounders. Conversely, a higher score for consumption of percent saturated fat (HRQ4 = 1.14, 95% CI: 1.06-1.23, p-trend=.001) was significantly associated with higher rates of incident RA.&#13;

Conclusion:&#13;
