The Impact of Resection Status and Tumor Site on Glioblastoma Survival

Abstract
Background: Glioblastoma (GBM) is the most common and the most aggressive primary malignant tumor in the adult brain. Patient survival remains poor despite treatment with 5-year survival rates near 10%. Prognostic factors include age, performance status, specific markers (MGMT methylation, IDH mutation), and likely the extent of resection. It is unknown whether tumor location or resection status at each cerebral site differentially predicts survival. Methods: All patients with recurrent glioblastoma treated at Rhode Island Hospital between 2005 and 2021 were identified from the Lifespan Cancer Center database. Demographic information, as well as tumor location at presentation, survival time, and resection status were computationally extracted from patient charts. Differences in survival based on various tumor locations were analyzed with Kruskal-Wallis and Kaplan-Meier statistics, and outcomes based on resection status within each tumor location were analyzed using the Wilcoxon rank sum test. Results: The analysis included 863 glioblastoma patients. There were no significant differences in survival between cerebral locations. Overall, resection significantly improved survival time (p<0.001). Analysis of specific tumor sites revealed that resection significantly improved survival in frontal (p<0.0001), parietal (p<0.0011), temporal (p<0.0001), and overlapping (p<0.0001) tumors. There was no difference in survival based on resection status for occipital, cerebellar, or deep brain structure (basal ganglia, thalamus, insula) tumors, and the sample sizes were too small to evaluate tumors of the ventricles or corpus callosum. Conclusions: There is a survival benefit from resection of frontal, parietal, and temporal GBMs, as well as those in overlapping lobes, but not for occipital or cerebellar tumors. These data may be used to stratify patient prognosis and optimize decision making on neurosurgical resection.
