JOINT EFFECT OF RHEUMATOID ARTHRITIS AND DIET QUALITY ON CARDIOVASCULAR AND MORTALITY OUTCOMES: INSIGHTS FROM THE WOMEN’S HEALTH INITIATIVE

Abstract:
**Objectives** This study aimed to assess how diet quality and rheumatoid arthritis (RA) together affect the risk of cardiovascular events and mortality among postmenopausal women. **Methods** In the Women's Health Initiative, 99,840 women without RA at baseline and 10,494 women with RA at baseline were followed prospectively for an average of 12.7 years. Diet quality was measured using food frequency questionnaires and the Healthy Eating Index 2015. Cardiovascular events and mortality were tracked during the study. Four groups were analyzed: a Reference Group (no RA and healthy diet), Group 1 (no RA and unhealthy diet), Group 2 (RA and healthy diet), and Group 3 (RA and unhealthy diet). Data was analyzed using multivariable Cox proportional hazards models. **Results** After adjusting for confounding factors, Groups 1 and 3 showed significantly higher rates of cardiovascular disease (CVD), coronary heart disease, and stroke compared to the Reference Group. All groups had higher CVD-related and all-cause mortality compared to the Reference Group. Among women with RA, diet quality did not significantly affect the incidence of cardiovascular outcomes. **Conclusion** A healthy diet is associated with reduced cardiovascular and mortality risks in women without RA. However, among those with RA, the protective benefits of a healthy diet are less pronounced, and RA itself appears to negate the potential benefits of diet quality on cardiovascular and mortality outcomes. Conversely, in women without RA, a healthy diet significantly reduces the risk of adverse cardiovascular events and mortality compared to those with an unhealthy diet.
