Evaluation of a Pediatric Integrated Behavioral Health Remote Patient Monitoring Program at a Health Systems-Associated Practice

Abstract:
Background: Behavioral health issues affect 12-27% of children; however, identifying and referring children for appropriate services can be difficult. To help pediatricians better screen for these concerns, Coastal Medical launched the ""Pedi IBH RPM"" program in June 2022. This remote patient monitoring program provides age-specific resources alongside a platform for communicating physical and social health concerns between annual pediatric wellness visits. Objectives: 1. To understand engagement with the program and assess needs outside of usual office-based care. 2. To identify opportunities for improvement to increase engagement and satisfaction among patients and their parents/caregivers at Coastal Medical pediatric offices. Design: Mixed-methods design employing survey administration, chart reviews, and literature review. Setting: Four Coastal Medical pediatric clinics across Rhode Island: Toll Gate, Waterman, Narragansett, and Bald Hill. Participants: Parents/caregivers of patients aged 3-12 attending their annual wellness visit. Instrument: SMS text messages, PRiSM Portal, Microsoft Excel, eClinicalWorks, SurveyMonkey, PubMed, Google Scholar Intervention and Evaluation: Parents were invited via text message to join the program one week after their child's wellness appointment. Follow-up check-ins were sent at one- and six-month intervals after their last wellness visit. Chart reviews were conducted to determine the types of concerns parents reported. Results: Out of 6,381 patients offered to join the program, 1,327 (20.79%) enrolled. Among the enrolled, 722 (54.41%) engaged with the surveys; of those, 140 (19.39%) were identified as having opportunities for intervention. The majority of parents reported social concerns (N = 89). The program was rated 4.6/5 by patients in the satisfaction survey. However, parents also noted a lack of knowledge about the program. Conclusion: Coastal’s Pedi IBH RPM program was successful overall, and patients indicated high satisfaction. Engagement can continue to be improved by reframing the survey questions, increasing screening frequency, improving marketing in the clinic setting, and adding more resources to the resource library.
