The relationship between food insecurity and gaps of care in diabetic patients at FQHC

Abstract:
Gap in care occurs when the patient is not getting the care provided that aligns with the recommended best practices in healthcare. This is a problem because the patient may continue to live with certain diseases unknowingly of their risk level and miss timely screenings and preventative care. This problem can be exacerbated by a patient’s – Social Determinants of Health. Analyzing this problem for the patient population at EBCAP Health Center will help find ways to better understand and tackle both the SDOH and close care gaps. The goal of this project is to understand the relationship between the SDOH and Gap of Care by looking at EBCAP patient population with SDOH and Gap of Care between 2022 and 2023. Furthermore, a subset of population with SDOH, specifically patients with gaps of care related to diabetes will be analyzed separately to better understand the effect of SDOH, especially food insecurity from the rising cost of food and rent prices nationwide.
