Supervised Exercise Therapy After Ablation of Persistent Atrial Fibrillation: A Prospective Randomized Controlled Trial

Abstract
Current evidence shows that there is a correlation between improved cardiorespiratory fitness (CRF) and a reduction in atrial fibrillation (AF) burden and symptoms. However, patients that have undergone AF ablation are typically excluded from these studies so the benefits of CRF improvement post-ablation is unclear. In a randomized controlled trial (RCT) for patients with persistent Atrial Fibrillation (AF), we aim to determine if a 6-month supervised exercise therapy (SET) program post-ablation would decrease hospitalization rates and AF burden at 12-months follow-up in patients who successfully complete the program. Eligible and interested patients who agree to participate will be randomized to either standard of care (SOC) follow-up treatment or to a 24-week long SET program. Patients will either receive an implantable loop recorder or a portable event monitor to assess for AF recurrence. Additional measurements that will be compared pre/post SET include echocardiogram measurements, inflammatory markers, body mass index (BMI), and CRF (peak METS, Borg Scale of perceived exertion). Planned enrollment in this feasibility study is 30 patients, with 26 patients currently enrolled. Data collection is ongoing, but this study aims to establish feasibility for adherence to the protocol and provide preliminary evidence of a potential correlation between CRF improvement and reduced AF recurrence post-ablation. Preliminary findings from this study will be used to support a larger, more comprehensive RCT examining the efficacy of a 6-month SET program compared to SOC in reducing AF recurrence post-ablation and associated morbidity and healthcare utilization.
