Abstract:
By implementing an ED, nurse-driven assessment protocol focused on the functional and social determinants of geriatric health that impact recovery from an acute episode, hospitals and emergency providers can improve quality, decrease costs and potentially generate new revenue. My CCP focuses on making such a program reproducible, scalable and financially sustainable for community hospitals. By simplifying the implementation and standardizing the assessment protocol, both single site facilities and large health systems can demonstrate clinical and financial benefits with this program.
