Causes of Colorectal Cancer Screening Disparities at Providence Community Health Centers

Abstract:
Intro: Colon cancer kills approximately 881,000 people annually with better outcomes with early detection. Some groups, facing barriers to receiving colorectal cancer screening (CRCS), are at a higher risk. In Rhode Island white patients are 5% more likely to receive CRCS than African Americans and 15% more likely than Hispanic/Latinos. Other barriers might include other social determinants of health (SDOH). The goal of this study is to assess barriers to CRCS in the Providence Community Health Centers (PCHC) population. Methods: A retrospective chart audit of all patients between the ages of 50 and 75 who were seen at PCHC between 12.1.2018-12.1.2019. Data gathered included: whether a CRC screening was ordered, demographics, and SDOH screening findings. Data was analyzed using StataSE and Excel. Results: 45,443 patients visits were reviewed for a population of 9,342 patients. Of patients due for CRC screening 26.9% had the order placed at that visit. Non-English-speaking patients were 8.2% less likely to receive CRC screening than their English-speaking counterparts (X2(27) = 674, P<0.001). Those who identified as Latino/Hispanic were 8.8% less likely to be screened than their Non-Latino counterparts (X2(1) = 148, P<0.001). Patients who screened positive for Social Determinants of Health (SDOH) were 3.4% more likely to have a screening ordered than those who did not. Discussion: SDOH, particularly language barriers negatively impact CRCS at PCHC. Interventions aimed at further understanding and overcoming these barriers are needed.
