Mobile Health Technology for Colorectal Cancer Screening: Pilot-testing E*Navegante, a Colorectal Cancer mHealth Tool, for Use in Primary Care Settings

Abstract:
With the increased availability of mobile technology, there exists and opportunity for electronic-based patient education programs to help start conversations between providers and patients in which the patient feels informed and prepared to make their own decision. We created the e-health tool E*Navegante to educate patients about the importance of colorectal cancer screening and different methods of screening. The tool, developed in both English and Spanish with the help of a local federally qualified health center and several other key informants, combines text, audio, and images in order to reach those with low health care literacy levels. To understand patient's perceptions of E*Navegante, individual interviews were conducted among English and Spanish speaking patients who were eligible for colorectal cancer screening according to the United States Preventative Services Task Force guidelines. Between November of 2018 and February 2019, a convenience sample of 6 English speaking and 6 Spanish speaking participants were recruited from a federally qualified health center and community-based organization to participate in approximately 20 minute long one-on-one interviews with investigators about the effectiveness of E*Navegante. Participants were given the tool to use on the clinic's tablet and then asked questions about the tool's usability and content. Themes from interviews were identified using the Immersion/Crystallization method and appropriate edits were made to refine E*Navegante. Overall, both English and Spanish speaking participants understood the purpose of the program to be educating others about the importance of colorectal cancer screening, the various methods to be screened, and the importance of living a healthy lifestyle. Only 2 participants used the audio component of the tool, thus prompting us to make the feature more prominent. Additionally, participant's suggestions for grammatical fixes, technical problems, and a summary page were taken into consideration and edited. With the tool refined, the next steps will be to pilot test the tool to determine the impact on screening rates among a diverse patient population.
