Barriers to Completion of Pediatric Developmental Screening in a Federally Qualified Health Center

Abstract:
Background: An essential part of pediatric doctors’ appointments is evaluating developmental milestones. Studies have shown that if interventions are provided early enough during these developmental appointments, children have a better chance of understanding and getting treatment for chronic conditions. Thundermist, a prominent Rhode Island Federally Qualified Health Center, has struggled to increase developmental screening rates. Objectives: The project aims to conduct a quantitative analysis, using chart audits, of children attending the Thundermist Clinics in Rhode Island to determine what the most significant factors are contributing to the failure to perform developmental screening at the recommended intervals. Methods: Study participants included all children who attended the Thundermist Health Center and are non-compliant. Internal quality improvements software was queried to determine which children receiving care at Thundermist's Woonsocket site were out of compliance (N=74). A chart review was conducted to determine why their most recent screening was not completed. Results were categorized by reason and analyzed using statistics. Results: Most commonly, surveillance was not conducted because the care team did not administer a developmental screening in the correct window (N=34, 46%). Less commonly, a visit was conducted, but the improper screen is administered (N=6, 8%), the patient’s no-showed (N= 14, 18.9%), and patient cancellations (N=9, 12.2%). The least common reason is the patient rescheduled and the facility canceled (N=1, 1%). Conclusions: Thundermist could further the results from this study by holding a provider meeting to update everybody about the standards that need to be met, increasing compliance by two-thirds. Proactive outreach to families due for screening may reduce missed opportunities due to canceled or no-showed visits.
