MRI Segmentation of Patient-Specific Anatomy to Assess Treatment Efficacy of Tumor Treating Fields for Glioblastoma

Abstract:
Glioblastoma (GBM) is a Grade IV malignancy originating from glial progenitor cells and comprises about half of all primary malignant brain tumors. GBM has a high mortality with a 2-year survival rate of 18%, 3 year survival rate of 11%, and 5 year survival rate of 5%. In 2015, the FDA approved tumor treating fields (TTF), a clinical treatment which utilizes electrical transducer arrays to impair tumor cell mitosis TTF provides superior survival outcomes compared to conventional chemotherapy with the added benefit of a significant decrease in the frequency and intensity of adverse reactions such as vomiting, nausea, constipation, diarrhea, anorexia, pain, and fatigue. It is currently unknown how patient-specific neuroanatomy affects electrical field delivery and whether there is a dose-response relationship that corresponds to marked improvements in patient survival. In this project, I hope to optimize a semi-automatic workflow utilizing MRI segmentation and finite element analysis to assess whether a dose-response relationship exists in GBM TTF therapy. The goal for this SC project is to generate mesh contours for 6 patients: comparing 3 that responded to treatment and 3 that did not. In this poster, I present the methodology for contouring one patient.
