3D Quantitative Evaluation of Temporalis Muscle Atrophy Following Decompressive Hemicraniectomy

Abstract:
Introduction: Temporal hollowing and atrophy are common complications following decompressive hemicraniectomy (DHC) due to intraoperative transection of the temporalis muscle. These complications compromise facial aesthetics and cause significant functional impairments, often necessitating further surgery. This study aimed to quantify temporalis muscle using postoperative CT imaging. Methods: We conducted a retrospective study on all patients at our institution who underwent DHC and possessed CT imaging at least six months post-DHC. Temporalis muscle dimensions, including volume, surface area, length, and height, were measured and compared between operative and nonoperative muscles. Flatness and elongation, indicating flattening and muscle stretching, respectively, were included to assess clinical relevance. Statistical analyses were adjusted for confounders including age, sex, smoking status, and hypertension. Results: Among 100 patients included, significant reductions in temporalis measures were observed on the operative side. Muscle volume decreased by 33.6% (p < 0.001), surface area by 39.0% (p < 0.001), flatness by 16.6% (p < .001) and elongation by 5.03%, (p < 0.05). Length decreased by 26.9%, (p < 0.001), and height decreased by 29.5% (p < 0.001). Height above the zygomatic process, a region significantly affected in temporal hollowing, was reduced by 42.7%, (p < 0.001). Conclusion: Our findings reveal morphometric changes post-DHC impacting jaw function and facial symmetry. These changes cause debilitating functional impairments and frequent surgical interventions. Addressing these issues is critical to preventing long-term disabilities and psychological distress.
