- The treatment approach for advanced cancer has been evolving rapidly with the introduction of novel targeted therapies and immunotherapies. Despite the rapid expansion of therapeutic options for advanced cancer, little is known about the real-world impact of these new treatments, particularly among older adults. This study aims to examine the real-world use, effectiveness, and cost of cancer therapies that are commonly used for older adults receiving care in routine oncology practice. We investigate a population-based sample of older adults with advanced cancer in the U.S. using the Surveillance Epidemiology and End Results-Medicare linked data. The first chapter of this dissertation examines trends in treatment patterns, survival, and spending for older adults with advanced non-small cell lung cancer after the introduction of immune checkpoint inhibitors. The second chapter examines the characteristics and prognosis of older adults with non-small cell lung cancer treated with immune checkpoint inhibitors in routine practice. The third chapter determines whether well-designed observational studies can correctly infer cancer treatment effects by comparing the effectiveness of bevacizumab plus chemotherapy versus chemotherapy alone among older women with advanced breast cancer. Our findings can generate important evidence on the expected clinical and financial impact of common treatment options, enable patients and providers to make informed decisions, and advance evidence-based cancer care.
