Blebs of hope: searching for markers of preeclampsia in placental exosomes

Abstract:
The practice of liquid biopsy is increasing in clinical significance in staging, typing, and tracking tumor changes in cancer patients. The isolation of tissue-specific genetic information from blood is facilitated by the isolation of small extracellular vesicles (30-150nm), called exosomes, which contain mRNA and miRNA, and tissue-specific surface markers. Sequencing of exosomal RNA increases understanding of the transcriptional profile of individual tissues types. This is an important advance that helps clinicians guide treatment based on genetic information ascertained from a simple blood sample. This study aims to determine whether liquid biopsy techniques can be used in the diagnosis of pregnancy complications. For this study we recruited 30 patients from Women & Infants Hospital in Providence, Rhode Island; 15 with early onset severe PEC (<34wk gestation; sBP >160mmHg or dBP >110mmHg; or PEC with at least one symptom of systemic involvement) and 15 gestational-matched, uncomplicated pregnancy controls. Exosomes were isolated from 8ml blood samples by differential centrifugation with all samples verified by a Nanosite bioanalyzer. mRNA was isolated from exosome samples and complementary cDNA was synthesized for high-throughput sequencing. Sequencing reads were mapped to the human genome using Tophat2. Preliminary data suggests expression differences in many genes when samples from control patients are compared with samples from patients with severe, early-onset preeclampsia
