- Traumatic brain injury (TBI) is a major cause of death and disability in the US, leading to 2.5 million TBI-related ED visits, 282,000 TBI-related hospitalizations, and 56,000 TBI-related deaths. Timely identification and treatment in TBI is crucial to patient outcome. To estimate intracerebral hemorrhage volume, the current standard of measurement is ABC/2, which has been validated exhaustively but displays significant variability in precision with bleed phenotype. Recent advances in 3D illustration (3DI) allow physicians to precisely quantify intracranial pathology on CT. This work sought to validate 3DI as a viable method to measure hemorrhage and edema related to contusions. Baseline CT scans collected during the ProTECTIII multicenter clinical trial were reviewed retrospectively by a neuroradiologist (n = 881). Cases in which the largest traumatic lesion was due to a contusion were selected. Volume of hematomas were calculated using the ABC/2 method by a neuroradiologist. The largest cerebral hemorrhages and sections of edema associated with the contusions were segmented by a research assistant blinded to ABC/2 values using 3D Slicer. Results have not been analyzed, but will be done via Bland-Altman. Although 3D Illustration is currently more time consuming than simple ABC/2, machine learning integration could lead to instantaneous identification of certain bleeds, and could potentially serve as an important triage tool in ED radiology departments..
