- Abstract:
- Purpose To compare the accuracy of three commonly used biometric formulae across different axial lengths (AL) in a teaching hospital setting. Setting A single Veterans Affairs Medical Center (VAMC) Design Retrospective chart review from November 2013 to May 2018. The postoperative follow-up period was from 3 weeks to 4 months. Methods One eye of each patient who underwent cataract surgery with a monofocal intraocular lens (IOL) was included. The Holladay 2, Barrett Universal II, and Hill-Radial Basis Function formulae were used to predict the postoperative refraction for all cataract surgeries. We calculated the Mean Prediction Error (ME), Mean Absolute Prediction Error (MAE), Median Absolute Prediction Error (MedAE), and the percentage of eyes that had a prediction error of +/- 0.25 diopter (D), +/- 0.50D, and +/- 1.00D for each formula. We performed subgroup analyses for short (AL<22.0 mm), medium (AL 22.0-25.0 mm), and long eyes (AL>25.0 mm). Results Of the 1131 patients screened, 909 met the study inclusion criteria. We found no statistically significant difference in predictive accuracy among the three formulae over the entire AL range (p=0.69) or in the short (p=0.97), medium (p=0.75), and long (p=0.91) eye groups. The Hill-RBF formula had the lowest MAE across all ALs. The MAE was highest in short eyes (>0.5D) for all three formulae. Refraction error rates were comparable to those in non-teaching hospitals. Conclusion The three formulae produced comparable outcomes in IOL implantation during cataract surgery for all ALs. Short eyes posed the greatest challenge to predicting postoperative refractive error.
