Network dynamics of cognitive control and mood during deep brain stimulation for depression

Abstract:
The research aims in this dissertation are a part of a broader effort to provide new insights on neural mechanisms during different brain states in depressive networks, and how we can causally manipulate these neural signals to identify circuit-specific mechanisms underlying improvement of psychiatric symptoms with deep brain stimulation (DBS) therapy. Chapter one and chapter two outline in detail how existing frameworks for seizure monitoring in subjects with refractory epilepsy can be adapted for patients with depression to build an “NMU” or neurophysiology monitoring unit. Chapter three and four demonstrate quantifiable neural and behavioral characteristics and their response to DBS in limbic hubs. Specifically, chapter three provides novel insights on neural responses between two canonical DBS targets used for psychiatric indications during resting state. Chapter four first explores neural mechanisms during task states without stimulation. I subsequently connect our findings during resting state with stimulation, and during task states without stimulation to characterize neural responses to DBS in subjects with depression during task states, specifically during exertion of cognitive control. This chapter centers cognitive systems or cognitive control as the behavioral domain of interest in depression.
