Purpose: This study investigates the association between frailty, as measured by the modified five-item frailty index (mFI-5), and outcomes such as inpatient mortality and hospital length of stay in geriatric patients presenting with traumatic fall injuries. Despite the established role of falls as major contributors to morbidity and mortality in those over 65, the interaction between frailty and post-fall outcomes remains underexplored. Methods: Data for patients aged 65 and above, admitted between 2014-2020 to Rhode Island Hospital's trauma service for fall related injuries, were extracted from its Trauma Registry. Frailty scores, based on mFI-5, were retrospectively assigned. Logistic and linear regression analyses were utilized to investigate the relationship between mFI-5 scores and mortality, as well as hospital length-of-stay, respectively. Results: Of the 6782 patients (mean age: 81.7 ± 8.66 years), frailty distribution was as follows: 12.8% (mFI-5: 0), 36.6% (mFI-5: 1), 32.2% (mFI-5: 2), 14.6% (mFI-5: 3), 3.2% (mFI-5: 4), and 0.6% (mFI-5: 5). Elevated frailty correlated with higher inpatient mortality (OR, 1.259; 95% CI, 1.14-1.39; P<0.000) and longer hospital stays (Coeff., 0.460; 95% CI, 0.35-0.57, P<0.000). Interestingly, age was not significantly linked to mortality and showed a negative association with hospital stay duration. Conclusions: Higher mFI-5 scores were positively associated with increased inpatient mortality and prolonged hospital stays in geriatric patients with traumatic fall injuries. This suggests that frailty, as quantified by mFI-5, could serve as a more robust predictor of negative outcomes than age in this population, thereby providing valuable insights for risk stratification.
