Telehealth and Antimicrobial Stewardship: Framework during a period of rapid acceleration within primary care

Abstract:
Antibiotic resistance is a global public health threat and improving antibiotic use is a critical safety issue and a worldwide public health priority. The Centers for Disease Control and Prevention (CDC) has outlined Core Elements for Antibiotic Stewardship in hospitals, long-term care facilities, and even some outpatient settings; however, almost no guidance exists for alternative healthcare delivery models such as telehealth. With an explosion in the use of telehealth within primary care, since the COVID-19 pandemic, there is a critical need to identify novel antimicrobial stewardship strategies for primary care settings utilizing telehealth. The use of telehealth in primary care presents several unique barriers to antimicrobial stewardship, including limited physical examination and changes to patient-provider relationship, that must be overcome to ensure appropriate and safe antibiotic use in this setting. Our review proposes a tailored, sustainable approach to antimicrobial stewardship for primary care settings using telehealth based on the CDC’s four core elements of outpatient antimicrobial stewardship including commitment; action for policy and practice; tracking and reporting; and education and expertise. Improving antibiotic use within primary care settings utilizing telehealth is critical as telehealth will remain a priority despite whether the COVID-19 pandemic recedes, particularly within certain patient populations with limited access to healthcare such as those residing in rural locations and older patients with transportation challenges.
