Trends in Awake vs. Asleep Deep Brain Stimulation from 2010 - 2022: A NSQIP Review

Abstract:
Background: Current literature regarding deep brain stimulation (DBS) has debated the efficacy and safety of awake and asleep approaches; however, there are no studies analyzing the incidence rates of both procedure types through time. This study aimed to elucidate the trends in awake and asleep DBS by using the National Surgical Quality Improvement Program Participant Use Data File (NSQIP PUF). Methods: The CPT codes regarding neurostimulator electrode implantation with MER (61863, 61864) and without MER (61867,61868) were isolated from the NSQIP database from 2010-2022. Statistical analysis was performed in R studio. Results: 434 cases of DBS were isolated with 328 using general anesthesia (GA), 97 using monitored anesthesia care (MAC), 6 using local or regional anesthesia, and 3 using unknown or other anesthetic types. The most common operative implication of both groups was Parkinson’s Disease. There were no statistically significant differences in ASA scores (P=.139), length of stay (P=.540) or number of patients returning to the OR within 30 days of their initial procedure (P=.977) between patients receiving awake and asleep DBS procedures; however, there was a statistically significant difference in age at surgery between the two groups with patients receiving asleep procedures having a mean age 7.2 years below awake DBS patients (P=.001). Conclusion: The NSQIP PUF documented a higher proportion of asleep DBS procedures with age at procedure being the only statistically significant demographic characteristic. Hospital course as characterized by length of stay, return to OR at 30 days, and ASA score were similar among both groups highlighting similar safety profiles.
