- Background: Association between long-term habitual coffee intake and risk of coronary heart disease (CHD) among postmenopausal women is unclear. Objective: The objective was to examine the dose-response relationship between total, caffeinated, and decaffeinated coffee intake and risk of CHD in a large cohort of postmenopausal women enrolled in the Women’s Health Initiative (WHI). Design: A substudy (Observational Study) in 86,459 postmenopausal women within the Women's Health Initiative (WHI) with measured coffee intake were followed for an average of 11.5y for CHD events. The multivariable-adjusted models were computed by using Cox proportional hazards regression to evaluate the association between coffee intake and CHD. Results: Of the 86,459 postmenopausal women, 5,899 CHD events occurred. Drinking ≥2 cups caffeinated coffee was significantly associated with reduced risk of CHD in a dose response manner. After fully adjusting for potential covariates, drinking 2-3 cups of caffeinated coffee was associated with a 8% reduction in the risk of CHD (HR: 0.92; 95% CI: 0.86, 0.98) while ≥ 4 cups/d of caffeinated coffee intake was associated with 13% of reduction in CHD risk (HR: 0.87; 95%CI: 0.78, 0.96), p trend =0.001. Decaffeinated coffee intake showed no clear association with CHD; however, there was no increased CHD risk with higher consumption. Conclusion: Long-term caffeinated coffee intake (≥2 cups) may be beneficial on reducing CHD risk among postmenopausal women.
