- Essential Tremor (ET) is the most common movement disorder in adults. ET is thought to be caused by a dysfunction of the central tremor network, but little is known about the pathophysiology of this network. Deep Brain Stimulation (DBS) of the Ventral Intermediate Motor (VIM) thalamus has been shown to significantly reduce the amplitude, increase the frequency and decrease the regularity of hand tremor as well as alter other neurophysiological aspects of ET. Due to the known cerebellar dysfunction and cellular disturbances that occur in ET, multiple studies have looked at the effect of ET on finger tapping and other motor movements. However, there are a lack of studies exploring adaptive motor movement tasks in patients with ET. Therefore, we created a two-step behavioral task to examine the role of DBS on the cerebellar-VIM pathway in patients with ET. Because DBS is disruptive, we hypothesize that forward motor plans depend on communication from the cerebellum to the motor cortex, and so we predict disruption of this communication (by DBS) will lead to worsened performance on the two-step motor task. Further trials will be performed to gather enough data to assess the role of the cerebellum and VIM in ET
