- Normal Pressure Hydrocephalus (NPH) is a neurological disorder characterized by abnormal accumulation of cerebrospinal fluid (CSF) in the brain’s ventricles. It commonly presents with a triad of symptoms including cognitive impairment, gait dysfunction, and urinary incontinence, although not all patients will exhibit all three symptoms. Shunt placement can improve the incapacitating symptoms of NPH, with reported improvement rates varying between 30% and 85%. The high-volume tap test (HVTT) is frequently used as an invasive method to predict shunt surgery response. Our literature review revealed a gap in studies specifically detailing the clinical implementation of HVTT for predicting shunt surgery outcomes in NPH cases, thus highlighting the need for further research on this topic. This study conducted a statistical comparison of two groups of NPH patients with indeterminate surgical decisions evaluated at Rhode Island Hospital: those for whom an HVTT was ordered to assist in future decision-making, and those for whom it was not. Our objective was to explore the association between patient characteristics and the ordering of HVTT for making further surgical decisions. Our findings indicate an association between the decision to order HVTT for patients with indeterminate surgical decisions in NPH and the presence of specific comorbidities, including peripheral vascular disease, a variety of cardiovascular/pulmonary/metabolic conditions, and hypertension.
